To find out about your policy's acupuncture coverage, call the patient number on the back of your card and ask:
Do I have out-of-network coverage for acupuncture?
How do I submit super-bills?
Is there a cap on the number of visits allowed per year?
Do I have a deductible for out-of-network services? If so, how much of it has been met?
What percentage of the visit is covered?
Which diagnoses are covered under my plan?
