To find out about your policy's acupuncture coverage, call the patient number on the back of your card and ask:

• Do I have out-of-network coverage for acupuncture?

• How do I submit super-bills?

• Is there a cap on the number of visits allowed per year?

• Do I have a deductible for out-of-network services? If so, how much of it has been met?

• What percentage of the visit is covered?

• Which diagnoses are covered under my plan?
